Home and Community Services for the Elderly and Disabled in Livingston, Macomb, Monroe, Oakland, St. Clair, and Washtenaw Counties

About the Program

The MI Choice Waiver Program is a community based, in-home services program designed to help individuals stay in their own home or transition to an eligible community setting.

These settings include: a rented, owned or family owned home, an unlicensed assisted living home,Licensed Adult foster care, or a Licensed Home for the Aged.

MI Choice includes prescreening, assessment, support coordination, and direct purchase of services.

Who is Eligible?

Persons who:

    • Are medically appropriate for a nursing home
  • Are Medicaid eligible
  • Are 18 years of age or older with a disability who would otherwise require nursing home care
  • OR are individuals over age 65 and in need of nursing home care.
  • Have a monthly income of $2,313 or less.
  • Reside in the six-county area.

  • Transition supports
  • Counseling services
  • Environmental adaptations
  • Goods and services
  • Home delivered meals
  • Non-Medical transportation
  • Personal Emergency Response Systems
  • Private duty nursing
  • Respite Care
  • Self-Determination with fiscal intermediary
  • Specialized Medical Equipment
  • Training

This program does not provide 24 hour care.

How to Participate

All individuals referred to MI Choice receive a telephone intake interview to assist in determining eligibility for an in- home assessment.

If an individual is eligible, a registered nurse and social worker visit the person’s home for an in-depth assessment.

The assessment evaluates the Individual’s physical and mental health, functional status; informal and formal supports; home environment; financial resources; and third party supports, such as insurance.

If an individual is not eligible, he or she is assisted through referral to other appropriate community agencies.

Who Can Make Referrals?

MI Choice accepts referrals from seniors, persons with disabilities, family members, friends, physicians, hospitals, nursing homes, public service agencies, and other service providers.
